Liver Surgery

Liver surgery is a complex procedure indicated for removing cancerous tumors such as primary hepatocellular carcinoma or metastasis from colon or other sites; treating symptomatic benign tumors such as hemangiomas or adenomas; managing severe cirrhosis or liver failure often in need of transplant); biliary issues such as stones, strictures, cancer); vascular disorders and certain metabolic and congenital disease; with the aim to cure disease, improve survival, or enhance quality of life when other treatments have failed.

Major Indications for Liver Resection (Removal)

  • Malignant Tumors: Primary liver cancers (HCC, cholangiocarcinoma) or metastases (spread from colon, breast, lung, etc.).
  • Benign Tumors: Large or symptomatic tumors like hemangiomas, focal nodular hyperplasia (FNH), or adenomas.
  • Biliary Issues: Complex gallstones (hepatolithiasis) or bile duct cancers.
  • Vascular Conditions: Severe blockages or malformations.

Major Indications for Liver Transplantation (Whole Liver Replacement)

  • End-Stage Liver Disease: Cirrhosis from chronic hepatitis, alcohol abuse, or fatty liver disease.
  • Acute Liver Failure: Sudden liver shutdown.
  • Malignancy: Liver cancer that can't be cured by resection (e.g., meeting criteria like Milan) or certain other tumors.
  • Metabolic/Genetic Diseases: Wilson's disease, hemochromatosis, alpha-1 antitrypsin deficiency, etc.

Dr Paracha will evaluate your case, review imaging, labs, gastroenterologist consultation reports, etc and determine if you need referral to a tertiary care center with hepatobiliary specialist. For simpler cases, he can perform liver biopsies during exploration as well as resection of smaller tumors; he also does fenestration of large, symptomatic cysts.
